Neurological and physical evaluation services, namely, functional assessment programs for patients receiving medical preventative and rehabilitation services for purposes of guiding treatment and assessing effectiveness of preventative and rehabilitation therapies for the maintenance of balance and fall prevention; and providing preventative and rehabilitation therapy services, namely, neurological and physical therapies for the maintenance of balance and fall prevention
