Health care services, namely, providing one-on-one information about treatment options, support, counseling, and training to chronic kidney disease patients in need of dialysis; health care and disease management services provided to kidney disease patients, namely, monitoring and management of patients undergoing dialysis, including scheduling of appointments, monitoring of dialysis treatment, patient prescriptions and health data; collaborating with the interdisciplinary teams comprising treating physician, members of dialysis center staff, and the kidney care community to insure optimization of patient health and goals
