Registered Nurse providing public health teaching and one on one consultation, that includes, but is not limited to, head to toe assessment, gathering information and history (ex., medical history, social history, surgical history, allergies, medication list, living conditions, available resources, etc.), and working with the client to implement care plans related to their health care concerns and follow-up visits to re-assess for progress, no progress, or staying the same.
