Providing group and individualized support services, information, and advice for potential transplant patients, transplant patients, caregivers, and the families of transplant patients; Providing group and individualized support services and information for potential transplant patients, transplant patients, caregivers, and the families of transplant patients, namely, counseling, and physical well-being guidance; Providing group and individualized support services, information, and advice for potential transplant patients, transplant patients, caregivers, and the families of transplant patients about the transplant process and resources available for the transplant patient and family of the transplant patient; Providing group and individualized support services, and information for potential transplant patients, transplant patients, caregivers, and the families of transplant patients, namely, nutritional, stress management, exercise, sleep and other preventative care guidance; Providing group and individualized support services, and information for potential transplant patients, transplant patients, caregivers, and the families of transplant patients regarding the transplant process; Providing group and individualized support services, and information for potential transplant patients, transplant patients, caregivers, and the families of transplant patients, namely, educational meetings, and forums to discuss transplants and the transplant process
