An assessment service for mental health patients and clinicians enabling patients undergoing treatment to record their impressions of their physical, mental and functional condition at the initiation of treatment and at intervals thereafter and to periodically rate changes in condition by listening to their initial or subsequent recorded impression and evaluating changes in condition experienced since the initial or subsequent recorded impression
