Educational, advisory and mentoring services for patients with sub-acute medical conditions, while they receive care and as they transition between settings, to promote their knowledge concerning their conditions and their self management, namely, for managing patient medication, use of patient health records, scheduling and attending patient visits with health care providers and education about their conditions and symptoms
