Home health care services; Medical social worker services namely, long term care programs meals on wheels special transportation and community service programs; Home health care services, namely, geriatric health care, physical therapy, nursing services, therapy care, personal care; Physical rehabilitation; Rehabilitation patient care services which includes inpatient and outpatient care.

Personal care assistance of activities of daily living, such as, bathing, grooming, feeding and personal mobility; and respite care in the nature of short term or temporary non-medical personal care of the senior to provide relief to the regular caregiver, such as a family member, as necessary; all inclusive program services in the nature of delivering social work and personal care services in the nature of assisting with bathing, toileting, transferring, dressing and grooming and other aspects of personal hygiene, and assistance with activities of daily living; clinical social worker services, namely, coordinating medical, rehabilitative and therapeutic treatment and evaluating treatment effectiveness for senior patients.; Providing geriatric care management services in the nature of coordination of necessary services and personal care for seniors, namely, the coordination of home inspections, annual physical examinations, home health aide services, housekeeping services, elder companion services, emergency response system services, meals delivery and provision, adult day care, lifestyle and wellness program, dog walking services, and transportation for providing food, drugs, meals and personal transportation
