Educational services in the nature of learning centers featuring instruction in the field of educating individuals with intellectual disabilities and developmental disabilities on how to develop job skills and interests, and how to build community involvement and connections

(Based on Use) Providing congregate living facilities; Respite care services in the nature of providing temporary housing for dependent elderly or disabled people to provide relief for their permanent caregivers; Providing temporary accommodations, namely, residential homes and housing services for individuals with autism and other developmental disorders (Based on Intent to Use) Respite care services in the nature of adult day care

(Based on Use) Behavioral health services in the nature of crisis response to individuals experiencing a behavioral health crisis services; Behavioral health services in the nature of crisis response to individuals experiencing crisis related to mental health, substance-use, an intellectual disability, or a developmental disability services; Health care consulting in the field of connecting individuals with mental health disorders, substance-use disorders, intellectual disabilities, and developmental disabilities to community resources available for ongoing support of the individual's mental health disorder, substance-use disorder, intellectual disability, or developmental disability; Health care services, namely, providing nursing and behavioral support on a short-term basis to individuals transitioning from community hospital settings to residential settings; Health care services, namely, offering consultation and clinical support for providers and family caregivers supporting individuals at risk or in crisis due to mental health disorders, substance-use disorders, intellectual disabilities, or developmental disabilities (Based on Intent to Use) Mental health therapy services; Behavioral health services; Cognitive behavioral therapy (CBT); Behavioral health services in the nature of therapy for individuals with intellectual and developmental disabilities and their families or guardians services; Psychological care; Psychotherapy and psychotherapy counseling for children and adults in individual and family sessions; Psychotherapy featuring treatment with early intervention of autism, emotional disorders, or substance-related disorders; Psychological consultation; Health care consulting in the field of autism, emotional disorders, and substance-related disorders; Speech therapy services; Medical, physical rehabilitation and physical therapy services; Occupational therapy services; Health care services, namely, in-home medical services for children with acute or chronic illnesses; Health care services, namely, early intervention for infants and toddlers with a disability or developmental delay
