Medical spa services, namely, minimally and non-invasive cosmetic treatment therapies of the face and body; cosmetic skin care services; medical clinic providing weight loss solutions, services and programs, nutrition counseling, hormone therapy, including, bioidentical hormone replacement, anti aging therapy, and natural hormone therapy, medical aesthetic procedures, including, laser hair removal, laser peels, botulinum toxin treatments, microdermabrasion, liposuction, vein treatments, vein therapy, cellulite treatments, body contouring treatments, injectable filler treatments, facials, skin care and dermatology services, none of the foregoing offered in home health care, nursing home, hospice or addiction treatment facilities, or including or relating to home health care services, namely, mental health services, nursing services, physician services, occupational therapy services, physical therapy services, and speech therapy services, delivery of goods, namely, pharmaceutical preparations for persons receiving hospice care, receiving palliative care, at risk of nursing home placement or eligible Medicaid nursing home placement, hospice services, home or facility based palliative care services, home and community based services to functionality impaired elderly persons at risk of nursing home placement or eligible for Medicaid nursing home placement, namely, mental health services, nursing services, dental hygienist services, hearing and vision screening and treatment, physician services, occupational therapy services, physical therapy services, speech therapy services and home health services, managed health care services for persons eligible for Medicaid nursing home placement, home delivery pharmacy services for persons receiving hospice care, receiving palliative care, at risk of nursing home placement or eligible for Medicaid nursing home placement, case management services, namely, developing an individualized home and community-based care plan for persons eligible for Medicaid nursing home placement and coordinating for such persons the necessary medical services, mental health services, dental hygienist services, hearing and vision screening and treatment, occupational therapy services, physical therapy services, speech therapy services, home health services, transportation services and medical supplies, addiction treatment services, addiction treatment services in the field of drug and alcohol addiction, rehabilitation for substance abuse patients, rehabilitation of alcohol addicted patients, rehabilitation of drug addicted patients, and rehabilitation of narcotic addicted patients
