Home health care services; Hospice services; Medical services, namely, home and community-based coordinated care, including skilled and unskilled services, primary medical care, and hospice services to senior and/or disabled individuals, including individuals who are Medicaid and/or Medicare-eligible; Occupational therapy services; Physical therapy; Primary care medical services; Speech and hearing therapy services; Speech therapy services; Medical, physical rehabilitation and physical therapy services

In-home support services to senior persons, namely, geriatric care management services in the nature of the coordination of necessary services and personal care for older individuals; Providing non-medical in-home personal care services for assisting with daily living activities of persons with disabilities; Providing non-medical in-home personal care services for assisting with daily living activities of senior and/or disabled individuals
