Medical evaluation services, namely, functional assessment program for patients receiving medical rehabilitation services for purposes of guiding treatment and assessing program effectiveness; Rehabilitation of alcohol, drug and narcotic addicted patients; Rehabilitation patient care services; Mental health counseling and psychotherapy as it relates to alcohol, drug and narcotic addiction; Psychotherapy and psychotherapy counseling for children and adults in individual and family sessions
