Home-visit nursing care; Geriatric health care management services; Home health care services, namely, Services provided will include non-medical services including but not limited to: medication management and reminders, meal prep, and medical appointment reminders. In-home medical will include nursing services that require Registered nurses and Licensed Practical Nurses providing care that includes but not limited to: Blood pressure management, medication administration and care of activities of daily living.
