Case management services, namely, coordination of necessary legal, medical, physical, social, personal care and psychological services for children, disabled persons, the elderly, hospitalized patients and in-home medical patients; providing non-medical personal assistant services for others in the nature of planning, scheduling non-healthcare appointments, coordinating, arranging and assisting individuals to perform daily tasks; providing patient advocate and case management services, namely, coordinating the procurement of medical services and administration of medication; providing personal support services for patients and families of patients, namely, emotional counseling and emotional support
